It's about a car accident. The guy on the front passenger's seat of one of the two cars involved is stuck in the car and the firemen try to cut him out of the wreck with hydraulic scissors. He is seriously injured (his right foot is minced meat, he is feeling terrible pain) and Joshua who writes the blog and does his civilian service (I think) in a rescue service hears him say "please let me die". Joshua who realises that he means it strokes his shoulder as his mother had done when he was ill and feeling pain. Not in the excerpt is the end.
